RN Care Manager – Seattle, WA **This is a HYBRID role (2 days/wk on site, 3 days/wk remote), open to LOCAL and NON-LOCAL candidates,**
– Position / Specialty: RN Care Manager
– Schedule: 5x8hr, M-F, 080-1600 (2 days/week on-site at Beacon Hill clinic, 3 days/week working from home. Clinic will issue all necessary equipment to work remotely.)

– Years of experience REQ: 1 year in an Acute Hospital and / or Long Term Acute Care/Rehab/Skilled Nursing setting

– Locals accepted: Yes. This position is open to LOCAL and NON-LOCAL candidates.
SPECIAL NOTES
– Candidate is expected to have their own reliable transportation (NOT public transportation) for the duration of this contract.
– This Care management role focuses on transitions of care, including care coordination and medication reconciliation, with the goal of reducing ED visits and readmissions.
– All IP discharges, with the exception of labor and delivery and oncology follow-up.
– All ED visits, particular focus on patients with chronic conditions.
FACILITY HIGHLIGHTS
– Type of Facility: Outpatient Clinic (2 days/week on-site at Beacon Hill clinic, 3 days/week working from home. Clinic will issue all necessary equipment to work remotely.)
